We’re looking for a compassionate  Outreach Support Worker to join a dedicated team providing one-of-a-kind care to adults with a range of complex needs, including:

• Mental health conditions

• Substance misuse

• Anxiety

• Hoarding behaviours

• Learning disabilities

• Complex care

 Outreach Support Worker support individuals in their homes and communities, helping improve their well-being, independence, and quality of life.

What’s on offer Outreach Support Worker:
✔️ Daytime shifts only (Mon–Fri with occasional weekends)
✔️ 35 hours/week
✔️ Free DBS
✔️ Up to 28 days annual leave + your birthday off

What we need from  Outreach Support Worker:

• Experience in mental health or similar support role

• A strong “can-do” attitude and caring nature

• Good written, verbal & IT skills

• Flexibility

• Driver with access to a vehicle is essential
